Underaged state law requirements!

1. The Minor must be with parent or legal guardian! 

2. Minor must have school I.D or some kind of legal I.D with name and picture on it!

3. Minor must have their Birth Certificate with legal Guardians name on it, if not a Power of Attorney Letter explaining that they are the legal guardian of the minor!

4. The Legal Guardian must bring their I.D with them!

5. The legal guardian must be in the room while the procedure is being done!

6. There will be a 2 page form that must be signed by the legal guardian allowing us to do the procedure!

After that we are ready to rock!
